Can you help with our user survey?

Our vision is to be able to provide effective, local support to everyone who needs it. In order to do this we need to collect information about what people want, need and what the current gaps in service are.

By sharing your experience with us in answering this questionnaire you will help to ensure that the support we offer our clients is as effective as it can be.

This survey is aimed at people living in the UK. We would like to know about:

• you or your child
• your experience of living with or looking after someone with a condition, injury or marking that makes you/them look different
• about any support received
• what support works best
• any suggestions or recommendations you have.

Healthcare professionals

We are also interested in hearing from healthcare professionals. We share your aim of making sure that high quality care is provided as locally as possible so that best patient outcomes can be achieved.

We would very much appreciate your views, as an expert working with people living with conditions that affect their appearance, about patient needs and the services available to them. We would also value your opinion on what additional service or support would be useful to you regarding your patients.
