To leave a share of your estate:

To leave a specific sum:

I give free of tax to Changing Faces, whose registered office is The Squire Centre, 33-37 University Street, London, WC1E 6JN, Registered Charity Number 1011222 and Charity Registered in Scotland SC039725, the sum of (write in here whatever you wish to give) to be used by Changing Faces for its charitable purposes, and I declare that the receipt of its Treasurer or other proper officer shall be a full and sufficient discharge.
